Good Faith Estimate Notice

Under the No Surprises Act, Giovanni D. Giannotti MD, S.C. dba GDG Surgery Division provides Good Faith Estimates (GFE) for uninsured or self-pay patients upon request or scheduling.

What is a Good Faith Estimate?

• An estimate of expected charges for non-emergency items/services, including associated costs.

• Not a bill; actual costs may vary based on your care.

Your Rights

• Request a GFE in writing at least 3 business days before service.

• If billed $400+ over the estimate, dispute via patient-provider process or federal portal (1-800-985-3059 or www.cms.gov/nosurprises).

• Estimates valid for 12 months unless updated.
